A good IVF patient follow-up process runs for six months, not six days. Call on day 0, 3, 10 and 30, then about once a month to day 180, with short WhatsApp messages in between. Tie every touch to what the couple is actually waiting for, such as test reports, money or a family conversation, and save only the next touch as the enquiry's one pending callback.
Why IVF enquiries go quiet, and what is usually happening at home
When an IVF enquiry stops answering after the first call, the counsellor usually reads it as lost interest. It rarely is. A couple who has searched for a fertility clinic, filled a form and spoken to you for ten minutes has been thinking about this for months, sometimes years. Silence usually means something at home hasn't moved yet.
The common reasons, roughly in the order you'll hear them:
- Reports are pending. A gynaecologist has asked for tests such as AMH, a semen analysis or an HSG, and the couple won't book a consultation until results are in hand. Some tests are done on particular days of the cycle, so the next useful date can be three or four weeks away.
- The husband hasn't agreed yet. Often the wife enquires first. He may not have agreed to a semen test, or to the idea of treatment at all.
- Money is being arranged. IVF is a large expense for most families. Couples may be waiting for a bonus, an FD to mature, a loan approval or help from parents.
- The family doesn't know, or is divided. In many homes the in-laws have a say, and some couples have told nobody. A call at the wrong time, on a shared phone, can make things harder for them.
- They are comparing clinics. They have a quote from another centre and are weighing doctors, packages and travel time.
- They are recovering from a previous attempt. A couple whose last cycle didn't work may need weeks before they can talk about trying again.
None of these is a no. Each is a date in the future when the answer might change. The job of follow-up is to find out which reason applies, ask when it is likely to change, and be on the phone a day or two after that.

The stages an IVF enquiry moves through before a first consultation
Before the first consultation, an IVF enquiry isn't moving down a funnel so much as sitting in a waiting room. Your stages should say what the couple is waiting for, because that decides when the next call should happen. This list works for most centres:

Two rules keep this honest. Every open stage carries a next-callback date, and "Consultation booked" carries an actual appointment. The IVF patient follow-up process, day 0 to day 180
Day 0 is the day the enquiry arrives. Where a stage from the table above gives you a better date (reports due on day 12, salary on the 1st), use that date and slide the rest of the calendar along.

That is thirteen touches, eight of them calls. The first month is dense because that's when most couples are actively deciding. After day 30 the gaps widen, because calling a family every week while they save up money only teaches them to ignore you.
Why only the next touch goes in the diary
Don't schedule all thirteen touches the day the enquiry arrives. The calendar is a map; the diary holds only the next step. At the end of every touch, the counsellor writes one line about what happened and sets one next callback, usually the next row of the table, earlier if a trigger is closer.
This matters for two reasons. First, the plan changes after almost every conversation. If the wife says the semen report comes on day 14, the day-10 call becomes a day-15 call, and a pre-loaded list of thirteen reminders is wrong from the second touch onward. Second, one pending callback per couple means nobody calls them twice in a day because two reminders were left open, and nobody can quietly drop them either, because an enquiry with no next date stands out.
If a counsellor goes on leave, the next person opens the enquiry, reads the last comment and sees the one date that matters. Nothing sits in a notebook or a personal phone's reminder app.
What to say on the day-3, day-10 and day-30 calls
The first call has its own script, covered in the counsellor's first-call script and question list. These are the three IVF counsellor follow-up calls that decide whether an enquiry books or drifts. Many will happen in Hindi or a regional language; the tone should be the same in any of them. Calm, private, unhurried.
Always check it's a good time to talk. Many women take these calls at work or with family around, and "can I call you back at 6?" is a perfectly good outcome.
Day 3: the "did you get a chance to talk" call
Counsellor: Hello, am I speaking with [name]? This is [your name] from
[clinic]. We spoke on [day]. Is this a good time for two minutes?
[If no] No problem. When should I call back? [Note the time. Set it.]
[If yes] I had sent the doctor's details and the list of reports on
WhatsApp. Were you able to see them?

place before you come in? ... And when do you think that will be?The last question is the whole point of the call. Whatever date she gives, the next callback goes one or two days after it.
Day 10: the "what are we waiting for" call
By day 10, if they haven't booked, something specific is in the way. This call is about naming it, gently.
Counsellor: Hello [name], this is [your name] from [clinic]. Is now
a good time?
Last time you mentioned you were waiting for [the reports / to talk
to your husband / to sort a few things out]. How is that going?
[If reports] Have they come? If you like, send me a photo on WhatsApp
and I'll ask the doctor's team whether anything else is needed before
the consultation.
[If money] Shall I send you the consultation fee and what the first
visit covers, so you can plan? The treatment cost depends on what the
doctor advises after seeing you, so I don't want to guess a number
that turns out wrong.
[If family] Would it help if your husband, or anyone in the family,
spoke to me directly or came along to the consultation?
[If comparing] That makes sense. Is there anything I can clarify so
you can compare properly?Then set the next callback on the date of their blocker. Don't set "day 30" by reflex if the reports are due on day 13.
Day 30: the one-month review
Counsellor: Hello [name], this is [your name] from [clinic]. We spoke
a few weeks ago about a consultation with Dr [name]. Is this a good
time?
I'm calling to see where things are, not to push you into anything.
Are you still planning to see a fertility specialist in the next few
months?
[If yes] What has changed since we last spoke? Is there anything I
can help with?
[If they've seen another clinic] Thank you for telling me. If you'd
ever like a second opinion, you can call me directly on this number.
[If unsure] Would it be all right if I checked in with you once a
month? If you'd rather I didn't, just say so and I'll stop.That last question does more for your centre's name than any script line. Couples who say yes have agreed to hear from you. Couples who say no are closed with a reason, instead of being called until they block the number.
WhatsApp messages that keep the door open without pressure
WhatsApp has one rule every counsellor must know. When a patient messages you, you can reply freely for 24 hours from their last message. Outside that window, or if they have never messaged you at all, you can only send a template Meta has approved.
The second rule is your own: write every message as if someone else might read the notification. Phones are shared and previews show on lock screens. Keep "IVF", "fertility" and "infertility" out of the first line. "Your consultation at [clinic]" is enough.
Inside the 24-hour window
Free-form replies, after they've messaged you. Keep them short and answer the actual question first.
Thank you for sending the reports. I've shared them with Dr [name]'s
team. Please also bring any old prescriptions to the consultation.
Shall I hold a slot for this Saturday at 11?The consultation fee is ₹[amount] and includes the doctor's review of

call when it's convenient.Each one asks for a reply, and a reply reopens the 24-hour window for a real conversation.

Reading the signals: when to call sooner and when to pause
The calendar is the default. These signals override it.
Call sooner, the same day if you can:
- They message on WhatsApp, even just "ok" or "thank you". Reply inside the window, then call.
- They say the reports have come.
- They enquire again through another form, an ad or the clinic's main number. Make sure the repeat enquiry lands on their existing record so the counsellor who knows them gets it, not a new caller starting from zero.
- The husband or a parent calls in.
- They ask detailed questions about cost, or about paying in instalments (only discuss EMI if your centre actually offers it).
- They mention a date at another clinic. They are close to deciding, and you want to be part of that decision.
Slow down or pause:
- Three calls in a row go unanswered. Send one WhatsApp template, move the next call out by two weeks and mark the status "Not responding".
- Money is the issue and they gave you a date. Call after that date, not before.
- They mention a recent loss, a failed cycle or a health problem. Stop the calendar. Send one kind, personal message and set the next call a month out, or leave it to them.
- They are starting treatment elsewhere. Wish them well and close the enquiry with that reason.
- They conceived naturally. Congratulate them and close it.
- They ask you to stop. Stop, note it on the lead, and keep them out of every campaign list.

Frequently asked questions
How many times should an IVF counsellor follow up before closing an enquiry?
About a dozen touches across six months is reasonable for couples who stay polite but undecided, as in the calendar above. The count matters less than the reason behind each touch. Close earlier if they ask you to stop, start treatment elsewhere or say plainly they aren't going ahead. After day 180, move the still-open ones to a yearly check-in rather than deleting them.
What should a counsellor never say on an IVF follow-up call?
Never quote success rates, promise results or interpret what a report means medically. That belongs to the doctor. Don't use age or time pressure to push a booking, and don't ask personal questions when the patient is clearly not alone. If she asks a medical question, write it down, say the doctor will answer it at the consultation, and offer the earliest slot.
Why is the IVF consultation not booked even after several calls?
Usually because the calls are about booking and not about the blocker. If three calls end with "we'll let you know", ask directly what they're waiting on and when that will change, then call after that date. Also check the basics: is the counsellor calling at the time the patient said suits her, and has anyone ever spoken to the husband?
Should the same counsellor make every follow-up call?
Yes, wherever possible. Couples share things with one person that they won't repeat to a stranger, and a new voice on day 30 feels like a call centre. When the counsellor is on leave, a colleague can take the call, but they should read every comment on the enquiry first and introduce themselves as calling on the counsellor's behalf.
How do we know whether this follow-up schedule is working?
Group enquiries by the month they arrived, not the month they booked, and check what share of each month's enquiries reached a first consultation by day 30, day 90 and day 180.
